Membership with the Maricopa County Medical Society (MCMS) qualifies you to receive a rebate on your credentialing application. To become a member, please print and complete the MCMS application, and return via U.S. mail or fax.

Please note, not all applicants may be eligible for membership with MCMS. Applications will be accepted according to our Bylaws. To further inquire about membership, please call (602) 252-2015.
